1. Briefly describe the project. a.
What were your original objectives? |

Construction of Two Environmently friendly dry toilets in Bohol, Philippines Provide computers and connect Doljo Elementary School to the
Internet. Provide Free medicines to Panglao Village through the Government
Health Clinic and Dr. Cogo, serving over 24,000 residents. |

Panglao Island, Bohol, Philippines Doljo Village Toilet provides clean tiled toilets to 24 families
who never had a toilet. Computers and Internet service was installed at Doljo
Elementary School serving 400 students and
disadvantaged women of the village. |

3. How did Rotarians manage and oversee the project? |

Administer all money for the MG on Quicken Accounting system.
Back up all entries with receipts. Design the Toilet Blocks, Hire the construction company. Perform
weekly inspections of the project. Organize the PTA and Boy Scouts to paint
the finished blocks. Purchase geranic drugs at wholesale and deliver to Doctora Cogo’s office. Review the
patient list. Purchase all computers, and equipment. Have them installed by computer company and internet provider. |

Trustee-approved guidelines and that all of the information contained herein is
true and accurate. Original
receipts for all expenses incurred will be kept on file for at least three
years, or longer if required by local law, in case they are needed for auditing
